Our community teams provide specialist care for you in your home and will access support at the hospice when you need it.

Our Clinical Nurse Specialists will very often be your first point of contact with the hospice. Liaising with your GP, district nurse and hospital consultant if necessary, they will assess your needs and ensure the care we provide meets those needs.

The Multi-Disciplinary Team provides a range of services to help you to remain at home:

Our Hospice at Home Team of nurses and palliative care assistants provides practical and personal care. 


Supporting people who wish to die at home, the Hospice at Home Team is available between 8am and 8.30pm, seven days a week, to provide practical hands-on nursing care to meet your individual needs. 

You can rely on us – 90 per cent of people who choose our Hospice at Home Team live at home to the end of their life because that is their wish.

They came every day and did whatever Ann wanted them to do. It was whatever she was up for that day. And when I say every day I mean every day – even Christmas Day.
